Manajemen Perioperatif Jalan Nafas Sulit ( Difficult Airway Management)


Jalan Nafas Sulit (menurut ASA) adalah suatu kondisi di mana didapatkan tanda ventilasi inadequate yang tidak bias diperbaiki oleh ventilasi menggunakan masker atau saturasi oksigen tidak dapat dipertahankan di atas 90% atau dibutuhkan waktu lebih dari 10 menit untuk melakukan tindakan intubasi trakea atau jika seorang ahli anestesi terlatih membutuhkan lebih dari tiga kali upaya menggunakan laringoskopi konvensional untuk melakukan intubasi trakea.

       Difficult to intubate/difficult to direct laryngoscopy : 1.15 – 3.80 %

       Cormack-Lehane derajat 3 : 5%, dan derajat 4 : <1%

       Difficult to ventilate : 5 % --> 50%nya disertai dengan diff to intubate

       Operator:

       Evaluasi yang kurang memadai

       Persiapan yang kurang memadai

       Teknik yang kurang memadai

       Peralatan yang kurang memadai

       Pasien:

       Kondisi fisik pasien (anatomis)

       Trauma, dan penyakit lain

MANAJEMEN INTRAOPERATIF

Persiapan

Persiapan pasien

       KIE pasien

       Kumur-kumur menggunakan lidocaine 1%

       Diberikan obat tetes hidung

       Dilakukan penandaan pada membrane cricotiroid

Persiapan tim

Pastikan tim anestesi siap

Persiapan alat dan obat

       Pastikan mesin anestesi berfungsi dengan baik.

Basic

Ø  S: scope (stetoscope, stetoscope precordial, laryngoscope Macintosh 3,4, McCoy)

Ø  T: tube ( ETT no 7;6.5;6)

Ø  A: airway device (OPA no 3;4;5, siap masker berbagai ukuran, siap Magill dan tampon)

Ø  T: tape

Ø  I: introducer/stilet atau bougie

Ø  C: connector

Ø  S: suction (connecting suction, suction catheter minimal 2)

Advance

Ø  Supraglotic airway device (LMA, LMA Fastrach)

Ø  Videolaringoscope (CMAC atau Glidescope)

Ø  Fiberoptic laryngoscope (FOL)

Ø  Bougie

Ø  Oksigen cadangan

Ø  Needle cricotiroidotomi

Ø  Jet insuflasi

Persiapan obat

Pastikan iv line lancar sebelum melakukan tindakan.

Emergency

       Ephedrine

       Sulfas atropine

       Lidocain

       Adrenaline
 

Anestesi

       Induksi: Propofol, Ketamine

       Analgetik: Fentanyl

Pelumpuh otot: Rocuronium, atau Vecuronium

Manajemen Postoperatif

Postoperatif pasien sementara tube in dan memerlukan perawatan di ICU

Planning

       Ventilator support

       Pasien sadar baik, diet sonde D5 50 ml tiap 4 jam, bila baik diet sonde cek retensi

       IVFD

       Support  hemodinamik

       Koreksi gangguan elektrolit jika ada

       Antibiotik

       Termoregulasi

       Cek laboratorium

       Monitoring VS dan balance cairan
